This Student Services Coordinator position will be exclusively focused to serve the students in Lexington School District Two, where 78% of students Healthy Learners service are Hispanic.  The Student Services Coordinator will assist the Midlands Community Manager with programmatic and administrative responsibilities and serve as a link between the student client, health care providers, parents, school and community personnel, especially the nurse.  The Student Services Coordinator must coordinate and communicate with all components in order to successfully serve the child.

Essential Functions

Health Screenings – plan and lead vision, dental and or other health screenings for Lexington Two students in school and other out of school time settings such as afterschool programs, summer camps and churches to identify children with health barriers to learning.

Care Coordination – work with families to obtain service consent and coordinate all needed healthcare services for the children to eliminate their health barriers to learning.  Appropriately document care coordination activities in the Apricot database which includes student medical appointments and follow-up care as specified by the provider.  Be a representative of and spokesperson for Healthy Learners.

Student Transport – serves as a primary person for safely transporting children to provider appointments.  Deliver glasses, medications and other resources to the student or school nurse in a timely and responsible manner.  Positively communicate and represents the Healthy Learners mission and vision.  Serves as a positive caring role model for students at all times.

Participates with other members of the staff in fulfilling the goals and objectives of Healthy Learners.  Assists with secondary office operations as assigned.

Qualifications

Experience working in a related professional environment required.  A four-year college or university degree in Social Services, Health Care, or an appropriate discipline preferred.  Fluency in Spanish is required, preferably as a first language.  Enjoys working with children.  Possesses a current license to operate a vehicle in South Carolina as well as a safe driving record and no evidence of offenses involving children.  Has knowledge of the geographic area of Lexington County.  Ability to work and interact well with others.  Demonstrated ability to expand own skills.  Ability to work independently.  Ability to prioritize assignments.  Solid organizational and time management skills.  Computer-literate with knowledge of computer software programs being used.  In the best interest of children, this position requires an extensive background check prior to employment.
