Healthy Learners AmeriCorps Member (Fall 2024)
The Healthy Learners AmeriCorps Member will assist the site staff with administrative responsibilities and serve as a link between the student client and community partners including health care providers, school personnel, and childcare providers. They may serve as a primary person for safely transporting children to provider appointments as well as for delivery of glasses, medications and other resources in a timely and responsible manner. The member will also appropriately document care coordination activities which include student medical appointments and follow up care as specified by the provider along with Healthy Learners intervention and plan.
